Our Jail Based Services team is seeking an experienced Case Manager to serve justice-involved clients who have been dually diagnosed with a severe and persistent mental illness as well as a substance use disorder, severe. This is a full-time position in Littleton, CO, working with the Douglas County Sheriff's office Monday - Friday supporting about 40 clients.

Job Purpose:
To promote efficient coordination of client care through collaborative practice and ongoing case management in order to improve care delivery and health outcomes.

Goals and Objectives:
* Meet productivity standards for the job role.
* Maintain an active caseload of clients as determined by department standards.
* Provide and document clinical care interventions to meet regulatory standards and/or contractual requirements.
* Improve client satisfaction and related health outcomes through delivery of person-centered treatment planning and collaborative care.
* Promote positive culture of collaboration within AllHealth Network.

Duties and Responsibilities:

* Conduct intake appointments with new WC applicants to determine their needs and design a case plan to work toward meeting these unmet needs.
* Assist the probation officer and problem-solving court coordinator to develop and implement an onboarding process for new WC clients.
* Assist WC clients to apply for social assistance programs as appropriate for each client, including but not limited to: SNAP benefits, housing vouchers and other housing assistance, SSI/SSDI, Medicaid and/or Medicare, Medicaid waivers, TANF, resources for individuals with developmental disabilities, and WIC. Help WC clients maintain these resources once secured.
* Assist WC clients to schedule mental and physical health appointments and attend these appointments with clients as necessary.
* Connect WC clients to educational and vocational opportunities and supports, and provide ongoing assistance to those clients as they pursue these opportunities.
* Develop and maintain a positive, ethical and professional relationships with community organizations, treatment providers, government agencies, and other resources of assistance to WC clients.
* Provide transportation assistance to WC participants as needed, including driving them to appointments.
* Partner with the probation department and the problem-solving court coordinators to identify and address other areas of need for WC clients.
* Maintain consistent communication with the WC probation officer, treatment providers, and the problem-solving court coordinator's office including providing updates to the WC team regarding clients' progress and struggles.
* Participate and contribute to departmental meetings, team huddles, rounds, supervision and other meetings, as needed, to ensure cohesive cross-organizational operations.
* Enhance professional growth and development through participation in specialized training.
* Perform case management services consistent with ethical and professional standards established by CMSA and disciplines pertinent to the employee.
* Follow all AllHealth Network policies and procedures.
* Complete all required trainings as listed in Relias Learning (both online training and face-to-face training) within required timelines.
* Perform other duties as required within the scope of the position and the experience, education and ability of the employee.

Key Technical Skills and Knowledge:
Education:
Bachelor's degree (Master's preferred) in social work, psychology, criminal justice, forensics, counseling or related field that encompasses case management skills in relation to dual disorders.

Experience:
Minimum of three years' experience providing case management services, preferably with criminal-justice systems.

Skills/knowledge:
* Demonstrated ability for assessment, evaluation, and interpretation of behavioral health information, and care planning.
* Knowledge of and familiarity with case management concepts, principles and strategies.
* Possess a high level of understanding of community resources, treatment options, funding options and special programs.
* Ability to operate PC-based software programs including proficiency in Word, Excel and PowerPoint and other computer based systems.
* Advanced written and oral communication, decision making and problem solving skills, efficient time management with multi-tasking ability.
* Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams.

Ability to meet minimum productivity and reporting requirements including but not limited to:
* Provide the WC probation officer and coordinator with any requested reports (i.e. mentor assignment, participant contacts, or participant transportation) on a weekly basis.
* Return all phone calls for the problem-solving courts coordinator and probation staff within 2 business days, unless taking time off as previously communicated to and approved by both the WC probation office and coordinator.
* Attend weekly staffing meetings as required by the judicial officer or the problem solving court coordinator.
* Maintain the highest level of program standards and open communication by attending and participating in regular WC team meetings and advisory board meetings.

Hourly Rate: $25.75 an hour

The base salary range represents the low and high end of the AllHealth Network salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The range listed is just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package detailed below.
