Job Description

JOB SUMMARY: The individual in this position is responsible for assessing, planning, implementing evaluating and documenting the management of selected and assigned group of patients requiring inpatient rehabilitation in the following age groups; adolescents, adult and geriatric. This care will be delivered in accordance with established standards. This includes working with physicians and other individuals from multi-disciplines to customize the rehabilitation program for inpatients, monitoring their progress relative to the guidelines for rehabilitation. Key duties include multi-disciplinary management of care, collaboration with the disciplines in providing patient/family teaching, patient/family support and counseling, continuous quality improvement, discharge planning and placement and securing appropriate resources at the time of discharge for all rehabilitation patients.

Manages the length of stay for inpatients needing rehabilitation utilizing multi-disciplinary practices, collaborative approaches with the physicians and program directors and socio-economic/clinical approaches within established policies and practices.

Directs multi-disciplinary patient care conferences utilizing functional independence measurement and CMS guidelines through multi-disciplinary discussion and intervention.

Directs and coordinates pre-admission and concurrent family conferences as necessary.

Assess needs for patients and family through multi-disciplinary input. Collaboratively designs an inpatient rehabilitation program to best fit the patients and families long term needs through timely interventions and education.

Proactively adjusts the rehabilitation program to meet the needs of the patient to meet established guidelines of rehabilitation therapy. Identifies situations of continuous quality improvement to enhance patient outcomes, assuring appropriate levels of care for the patient within the system and unit specific financial targets as appropriate.

Assists with concurrent reviews for inpatients promoting rehabilitation benefit as appropriate.

Prepares the patient, family and other team member for discharge placement assuring that the patient and others, if appropriate, have resources available or secured upon discharge.

Promotes rehabilitation throughout the continuum of care. Promotes inpatient rehabilitation programs and services to external customers and referral resources.

Actively participates in organization-wide care continuum approaches to patient care management.

Follows all CARF, Federal, Joint Commission and documentation processes and regulations.

Actively serves on ad hoc committees as necessary.

Review for approval all requests for charity care funding of equipment or services, assuring that this is the resource of last resort.

Education : Masters in Social Work

Licensure : PA Social Work license preferred. If not licensed at time of hire, candidate agrees to obtain LSW within 6 months of employment date.
