Qualified candidates for this position offer:

PURPOSE

According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Intake Supervisor, the Clinical Intake Coordinator coordinates client's transition from an institutional setting to home health care services, processes client referrals and initiates services.

ESSENTIAL FUNCTIONS

Clinical Review and Documentation
* Makes observations on an advanced level; report and records observations appropriately; forms nursing judgments and carries out nursing actions.

* Follows established standards of nursing practices. Assumes responsibility for compliance with federal, state and local regulations. Interpret existing policies to staff, families and physicians.

* Follows agency admission procedures and communicates admission to other departments, staff, physicians and other service providers.

* Initiate / edit verbal orders.

* Process complex referrals, i.e. wound care, risk for rehospitalization.

* Track inpatient discharges.

* First review of referrals where there are clinical questions or concerns.

* Enter face to face encounter information into Electronic Medical Record.

* Research/investigate Face to Face visit notes for edits and obtain missing information.

* Verify if primary diagnosis is correct based on clinical documentation and reimbursement

* Collects data base information during the intake process. Provides appropriate documentation of referral per agency policies and/or procedures.

* Obtains initial insurance authorization and assists case managers in tracking / obtaining additional information as needed.

* Communicates appropriate information to team members regarding new admissions and referrals, including client needs such as a plan of care, supplies, and equipment. Serves as a liaison between referral sources, interdisciplinary teams.

* Effectively assists the client, physician and/or discharging facility to establish an effective home care/hospice plan prior to the client's transfer to home setting.

* Updates referral sources on status of clients currently receiving service or recently discharged and the client's response to services received.

* Follows written and verbal directions

* Provides exemplary customer service to clients, peers, families, managers, and guests.

* Reports any observations found in clinical documentation on a timely basis* Follows all company, facility and state policies and procedures.
