Job Title: Rehabilitation Liaison Location: 1800 Orleans Street, Baltimore, MD 21287 Organization: Johns Hopkins Hospital Position Summary: Key Responsibilities: Evaluate patients to assess goals and needs for ACIR admission. Complete necessary documentation and coordinate patient admissions. Collaborate with multidisciplinary teams, including physicians, advanced practice providers, and care coordinators. Educate patients, families, and healthcare providers about rehabilitation goals and admission criteria. Contribute to marketing strategies to promote ACIR services within the community. Facilitate patient transfers and maintain detailed records of contacts and outcomes. Attend and actively participate in team meetings and discussions to improve workflows and address barriers to admission. Qualifications: Bachelor's degree in Nursing or Master's degree in Occupational Therapy, Physical Therapy, or Speech-Language Pathology. Active licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. At least three years of clinically relevant experience. Strong analytical, communication, and interpersonal skills. Proven ability to collaborate effectively in a multidisciplinary setting and demonstrate sound clinical judgment. Why Join Us? Salary Range: Minimum 41.09 per hour - Maximum 67.84 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
