Medical Social Worker - Inpatient Care Coordination
Overview Presbyterian is seeking a skilled and compassionate Social Worker to work PRN with the Inpatient Case Management Team in Care Coordination. Referrals for social work intervention require evaluation of needs to be met, understanding specific patient centered outcomes, and accountability of action within a case management team structure. Medical Social Workers assist with patient discharges from the hospital and coordinate care of patients with complex needs that require multiple services over time.as well as handling crisis intervention. Responsibilities * Collaborate with healthcare teams to develop and implement comprehensive care plans. * Assists with discharge planning, including arranging post-hospital services such as home healthcare, rehabilitation, or long-term care. * Provide crisis intervention and emotional support to patients and families. * Educate patients and families about available community resources, insurance benefits, and financial assistance.
