The Transitional Care Team (TCT) is an integral member of the TCT and co-leads in programmatic development and implementation. Founded on a human caring framework and utilizing relationship-based care principles, the model promises to transform practice and the care delivery for neurosurgical patients.
With recognition, the patient is at the center of the team, the nurse works collaboratively across the inpatient and outpatient clinical teams, managing, coordinating, and delivering transitional care services. Supplemental patient education will be provided in the form of a discharge bundle, streamlining the discharge process and further ensuring patients receive appropriate education, care and services to facilitate a seamless transition home. The Transitional Care RN will also play a vital role in access to care and help support the department and division in ambulatory clinical patient triage. May include weekend rotation. Education
