Overview

Medication History Pharmacy Technician

Full-Time; FTE 1.0

Monday-Friday; 8:00am-4:30pm + every 3rd weekend

Bettendorf, IA

Under supervision of a licensed Pharmacist, the Medication History Pharmacy Technician is responsible for completing the most accurate current medication list possible on newly admitted inpatients through information obtained from the patient, family, pharmacy, physician office, or other facility as required. Inputting correct home medication information, dose, frequency, time of last dose, primary pharmacy, and allergy information into the electronic medical record. Has excellent knowledge of brand and generic medication names, common dosages and dosage forms and frequency of administration of each medication.


Responsibilities
  • Interview newly admitted patient or patient's family to obtain the most accurate current medication list possible.
  • Contact patient's family, nursing facility, pharmacy, and/or physician office for medication clarifications as needed.
  • Obtain indications for PRN medications and actual frequency of use of PRN medication.
  • Prompt patient for additional medications by specifically suggesting over the counter (OTC) medications including herbals, vitamins, pain relievers, cold products, patches, lotions & creams, eye drops, sprays and inhalers.
  • Determine if patient was on recent antibiotics prior to admission.
  • Input most accurate current medication list possible into electronic health record.
  • Update patient's primary pharmacy and other pharmacies the patient may use in the electronic health record.

Qualifications

Education:

  • High School Diploma or Equivalent

Experience:

  • 2 years' experience as a Pharmacy Technician

License(s)/Certification(s):

  • Certified Pharmacy Technican (CPhT).
  • Pharmacy Technician license or eligible licensure in both IA and IL.
  • Most obtain secondary licensure within 90 days.
