Christiana Hospital in Newark, DE, a part of Christiana Care Health System is seeking an RN to work as a Diabetes Educator. In this position, you will work in various units and learn valuable skills.

To manage diabetes patients according to Diabetes Disease Management Protocols, provide diabetes education and coordination of services, and work in collaboration with all physicians, APC, nursing, and ancillary staff of the acute care inpatient areas. Coordinate and provide diabetes education patients for clinical acute inpatient areas. To provide patient education specific to the American Diabetes Association (ADA) guidelines and/or recommendations.

Participate in updating and maintaining related educational materials for ChristianaCare, ensuring compliance with the ADA recommendations for educational programs.

General Requirements:

  • Bachelor of Science in Nursing (BSN) required.
  • Masters degree preferred
  • Must possess or be able to obtain a Delaware State RN license or multistate compact RN license
  • Current BLS required.
  • Minimum of 5 years of nursing experience.
  • Experience working with at-risk populations is highly desirable.
  • Board Certification in diabetes care, or related field preferred

Primarily shift for this position will Day shift with Weekend and Holidays per the guidelines of the department. Hours: This position is 40 hours per week.

Principal duties and responsibilities

  • Demonstrates knowledge and skills necessary to provide care, education and support appropriate to adult and geriatric diabetes patients.
  • Obtains and interprets information to identify patient needs and provide the care needed.
  • Assess the learning needs of patients/families and carries out teaching as needed in regard to Diabetes self-management.
  • Provides the patient/family with information regarding the plan of care (diagnosis, medications, treatment, tests, diet, etc.). Develops and individualizes plan of care based on patient assessment.
  • Executes Diabetes Disease Management as per American Diabetes Association (ADA) Guidelines.
  • Maintains records that support requirements needed for ADA recognition.

Hourly Pay Range: $35.39 - $56.63 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
