Community Intervention Specialist - My Home


Department of Housing


Hourly Rate: $28.22 - $32.85



The Department of Health and Human Services Housing Division supports individuals with accessing and maintaining housing. We strive to provide Milwaukee County residents in need with a safety net, decent and safe affordable housing, and a connection to opportunities which improve their quality of life. We use available funding sources in the most equitable and efficient manner.



Job Summary: Under direction of the Program manager the Community Intervention Specialist My Home, maintains a case load of homeless and formerly homeless participants with targeted disabilities of severe and persistent mental Illness and substance use disorders. This position works to maintain participants housing stability, process annual paperwork to determine rent amounts, accurately input participant information in Yardi and HMIS databases, conduct Housing Quality Standards (HQS) inspections, and act as a liaison between case management, landlords, and tenants. The Community Intervention Specialist will also advocate on behalf of program participants.



Job Duties:



  • Manage a caseload of homeless individuals with the targeted disability of severe and persistent mental illness, and substance use disorders.
  • Act as a liaison between case management, participants, and landlords.
  • Advocate and intervene on behalf of client to maintain housing stability.
  • Input and verify participant information in applicable databases.
  • Complete Housing Quality Standards inspections of units.
  • Determine eligibility by certifying program participants
  • Attend applicable trainings and meetings.
  • Up to 70% travel within the community is required for this position.
  • Perform other duties as assigned.



Minimum Requirements:



  • Current resident of the State of Wisconsin at time of application
  • Current Wisconsin Driver's License and access to a reliable insured vehicle; both must be maintained throughout employment
  • Must be able to travel 70% of work time within the community
  • Bachelor's degree or higher in Social Work or a Human Services related field from an approved college or university
  • At least one (1) year of experience working with the homelessness population, in supportive housing, or related field.
  • At least intermediate experience with computer and software applications such as Microsoft Word and Microsoft Excel



Knowledge, Skills and Abilities: Knowledge of housing programs and opportunities as they relate to homelessness and individuals/families facing eviction. Knowledge of available housing resources; referrals and knowledge of the permanent supportive housing model; landlord evictions, and the ability to work with individuals with a wide range of housing prevention need. Ability to manage county funds that can be used for qualifying individuals.
