Job Summary
The Health Management Educator will provide telephonic education and support to members with chronic illness. The Health Management Educator will be responsible for improving health outcomes of and promoting appropriate preventive care for Medicare's members with certain scenarios such as Diabetes, Asthma, COPD, CAD, frailty, and risk of falls. This role will also include communication with health care professionals to ensure goals of a member's treatment plan are met, as well as ensuring that members are linked with the appropriate resources and are adopting healthy lifestyle practices to reduce risk factors. The Health Management Educator will identify participant's behaviors and provide customized health coaching to individuals based on their health risk level.

Essential Functions

  • The essential functions listed represent the major duties of this role, additional duties may be assigned.
  • Provides disease management and wellness coaching services to help control healthcare costs through education and program implementation.
  • Conducts assessments, surveys and develops a education plan and delivers health education to Medicare members.
  • Create a personalized action plan that will will include short and long term goals prioritized by the member, specific activities/interventions to achieve goals.
  • Assists the member and caregiver to understand their illness, make life style changes, adhere to medication regimen and work with their provider to develop a symptom response plan. Collaborate with multidisciplinary teams.
  • Identify and address barriers that are preventing the member from meeting their goals.
  • Counsel individuals on daily nutrition and medical nutrition therapy, including reinforcement of physician/nutritionist developed custom diet plans
  • Provide guidance and direction on accessing community based services for home modifications, transportation, meal preparation etc
  • Focus on prevention, health promotion, maintenance, restoration and education.
  • Develops strong relationships based upon trust with individual participants.
  • Assists clients in identifying, prioritizing, and implementing choices. Empowers clients to reach their goals, through direction, encouragement, accountability and counseling.
  • Provides advice and instruction for fitness, good nutrition, and management of weight, stress, and health into a participant's lifestyle
  • Commits to continued learning in all areas relevant to the position. Maintain and develop knowledge about various health topics and behavior change strategies.
  • Utilizes established workflows and collaborates with internal & external stakeholders such as: Member Services, Enrollment etc.
  • Complies with all policies and procedures to ensure that the program's outcomes measurements meet HEDIS and NCQA standards.

Required Work Experience:
3+ years related work experience. Experience Details: Working with chronic populations to provide health education, self management, wellness, care coordination support and lifestyle related topics.
Related Bachelor's degree required In Health Education is preferred. In health related field is acceptable.
Respiratory Therapist Certification

Preferred Qualifications:

Experience working in a health plan setting or doing predominantly telephonic health education and coaching is highly preferred

Experience working with the Medicare population is preferred Experience in health behavior change/health coaching/health education a plus

Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations

CDCES Certification

General Physical Demands:
Sedentary work: Exerting up to 10 pounds of force occasionally to move objects. Jobs are sedentary if traversing activities are required only occasionally.

Typical Annualized Hiring Range: $63,800 - $79,700

Annualized Salary Range: $63,800 - $103,600

Final pay will be determined with consideration of market competitiveness, internal equity, and the job-related knowledge, skills, training, and experience you bring.
