Provide in-dept nutritional assessment and follow up for patients at high nutritional risk. Develops care plan including discharge recommendations, which may include instructions on appropriate diet therapies to patients/family. Actively participates as core team member in interdisciplinary coordinated patient care program and on committees for the development and revision of nutrition related policies and procedures. May assess patients to determine level of care and implement care plan in patients of moderate or potential risk. Acts as a source of nutrition care expertise to MMC staff and associates.
Essential Functions
- Assess patients by evaluating nutritional status, determining level of care and implementing nutritional care plan. Nutritional status is evaluated through review of the medical record, patient/caregiver interview, and input from health care team
* Completed Assessment as evidenced in the medical record in adherence to policies and procedures, departmental and professional standards of practice, and time frames. - Completes comprehensive assessment of patients at high nutritional risk including formulation of care plan, goals and recommendations through continuum of care.
* Completes Comprehensive Assessment form including care plan that is realistic, appropriate and in conformance with professional and departmental policies and procedures and standards of practice. Additional evidence when recommendations acknowledged and/or implemented by health care team - Determines appropriateness of diet order and communicates recommendations for change when indicated.
* Documented evidence in medical record in conformance with departmental policies and procedures and time frames in making recommendations for change. Evidence that recommendations implemented by Physicians. - Monitors and evaluates efficacy of care plan in terms of patient compliance, tolerance, laboratory data, and physical and psychological.
* Completes progress notes in medical record according to departmental standards and time frames. - Evaluates patient/caregiver need for nutrition counseling. Provides counseling where necessary using method/materials to maximize comprehension and achieve desired outcomes including conformance with diet therapy and enhanced quality of life.
* Documented evidence of evaluation of need for nutrition counseling and its completion, if indicated, according to departmental and regulatory agency standards and time frames. Evidence presents of evaluation of patient comprehension and anticipated compliance. - Responds to nutrition consults within allotted time frames.
* Documented evidence in medical record of response to consult. - Documents, consults, initial assessments/care plans, comprehensive assessments, follow-up/progress notes, record. and counseling in medical record to provide record of intervention and recommendations.
* Evidence of intervention and recommendations in medical record. - Reviews nutrition office computerized patient file for accuracy of diet orders, evidence of prior intervention/recommendations in order to develop daily assignments and provide continuity of care. Documents intervention including assignment of care level, comprehensive assessment, counseling and recommendations on a daily basis in diet office computerized patient files.
* Documented evidence of adherence to departmental standards of practice in diet office computer file. - Records result of calorie counts as appropriate, according to departmental standards of practice.
* Documented evidence of calorie counts, pertinent information from meal rounds in medical record if indicated. - Provides dietitian coverage as staffing necessitates in order to insure timely response to nutrition consults and continuity of nutritional care.
* Provides full coverage as needed according to policy and procedures and departmental standards. Uses sound judgment in prioritizing consults and communicating coverage issues to dietitians and manager. - Participates in interdisciplinary conferences and departmental meetings to enhance patient care and provide nutrition expertise.
* Evidence through sign in sheets of attendance, on-line transcript and active participation in interdisciplinary conferences or meetings as scheduled. - Acts as a resource/source of nutrition expertise to other health care professionals including physicians, and nurses by providing inservices, lectures and instruction as needed/requested.
* Documented evidence of in-services/lectures in addition to evaluation by unit health care professionals as to quality of nutrition information provided. - Maintains current knowledge base through appropriate continuing education to maintain credentialing, current and professional knowledge and expertise.
* Attain average 15 continuing education credits per year (or 75 over 5 year period). - Educate, instruct and precept dietetic interns, AP4 students and Diet Technician students in keeping with departmental standards and the role of a major teaching institution.
* Measure performance of students in terms of standards, providing ongoing feedback to student and Director of Clinical Nutrition. - Actively participates as core team member in all aspects of coordinated care program.
* Attends daily meetings to communicate with team members implementation of nutrition assessment goals, progress and outcomes. - Assists in development and revision of diet manual, nutrition care guidelines, standards of practice and instructional materials. Participation may be by request or own initiative.
* Ongoing participation to achieve quality standard. - Participates in performance improvement initiatives including productivity evaluation, test trays and peer review in order to provide ongoing improvement in productivity, quality and continuity of patient care.
* Documents accurate daily activity log and completes performance improvement data/assignments as requested. - Is responsible and accountable for own actions.
* Conforms to Medical Center's human resource policies and procedures. - Actively participates, practices and promotes Montefiore excellence.
* Contributes to problems identification and problem solving in work environment. Supports the mission, vision, critical success factors and goals of Montefiore Medical Center and the standards of care. - Other duties as assigned.

Qualifications
Master's degree required for individuals who established eligibility for the registration examination effective on or after January 1, 2024. Baccalaureate degree in Nutrition or related field from an accredited college for those that established registry eligibility prior to January 1, 2024 Registered Dietitian (R.D.) status with the Academy of Nutrition and Dietetics (AND): Registration Eligible status acceptable conditional on the taking and passing of the next AND qualifying examination for Registered Dietitians (within 3 months). Maintenance of continuing education requirements mandated by the AND.
