Our Provisional Level 2 Trauma Center delivers comprehensive and specialized care for patients with traumatic injuries. The Stroke Center at NYU Langone Hospital-Suffolk is designated by the New York State Department of Health as a Primary Stroke Center, with expert neurologists available to provide treatment 24 hours a day, 7 days a week. Additionally, NYU Langone Hospital-Suffolk has multiple outpatient sites, including an outpatient wound care center, a hemodialysis center, and various primary care and specialty offices.

Position Summary:
We have an exciting opportunity to join our team as a Social Worker - Part Time, Days - Hemodialysis Hospital - - Monday - Friday with Alternating Weeken.

In this role, the successful candidate Assesses the needs of and provides assistance to patients and their significant others. Works with the interdisciplinary team to prepare a plan of care. Prepares and updates social work case records. Directs patients to appropriate community resources.

Job Responsibilities:

PROVISION OF DIRECT SOCIAL WORK SERVICES

  • Interviews patients/significant others to obtain psycho-social assessment.
  • Maintains social work narratives/documentation including nature of problem, short and long term goals, intervention and progress.
  • Makes home visits to provide support, social work services, bereavement counseling.

COMMUNICATES EFFECTIVELY WITH OTHER INTERDISCIPLINARY TEAM MEMBERS

  • Obtains necessary medical, nursing and other staff insights from appropriate personnel and/or medical records to assist in meeting and determining patient's needs.
  • Communicates with other team members exchanging information re: social, emotional and environmental problems, which may affect patients' response to treatment.
  • Attends and organizes case conference as needed to resolve problems or share information.

PROVIDES CONCRETE SOCIAL WORK SERVICES

  • Provides advice and guidance to patients to resolve social, emotional and long range financial problems associated with illness, disability, family conflict, etc.
  • Directs patients to designated community services for financial assistance, long term family counseling and so forth.

PARTICIPATES IN AGENCY WIDE PERFORMANCE IMPROVEMENT

  • Maintains current knowledge of social work trends in home care by reading appropriate journals and utilizing other resources.
  • Participates on the Performance Improvement Committee.
  • Assists in orienting and training new personnel and serves as a resource for other personnel in resolving patient care problems.
  • Attends monthly staff meetings.
  • Participates on departmental committees.
  • Exhibits a working knowledge of the departments' performance improvement functions by understanding and gathering data for studies and the like.
  • Complies with Department of Health regulations regarding care to patients and documentation of it as to not put the agency into a deficiency mode.
  • Accepts assignments based on demonstrated competency to provide care to age and gender specific population.

Minimum Qualifications:
To qualify you must have a Masters degree in Social Work.
Current New York State license to practice Social Work.
Prior experience in Community Health/Hospice.
Knowledgeable re: federal, state and local regulations.
Discharge planning experience.
Interpersonal skills.
Ability to provide own transportation. Must possess a New York State Drivers license.
Ability to read understand and interpret map..
