Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes.We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

The Member Engagement Agent plays a vital role in connecting with members, understanding their needs, and scheduling appropriate services to help them achieve better health outcomes. This role is the front line of our engagement operations-responsible for delivering high-quality, compassionate, and efficient outreach to members, while meeting or exceeding performance goals.

Responsibilities include:

  • Make outbound and receive inbound calls to engage members about available healthcare services.
  • Follow approved scripts to educate members, address questions, and encourage participation in programs.
  • Handle objections and provide appropriate rebuttals to support scheduling and service completion.
  • Schedule member appointments for in-home or telehealth visits.
  • Make reminder calls and send confirmations to reduce no-shows.
  • Meet or exceed outreach, conversion, and quality targets.
  • Adhere to all HIPAA, TCPA, and company compliance requirements.
  • Work closely with team leads, quality coaches, and managers to improve skills and outcomes.
  • Participate in ongoing training to maintain knowledge of programs, systems, and engagement techniques.

Qualifications and experience:

  • High School Diploma or equivalent experience
  • 1+ year of call center, customer service, or healthcare engagement experience preferred.
