Position Summary

The Physical Therapist is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional standards of practice.
You'll collaborate with an interdisciplinary team to coordinate care delivery and ensure the highest standards of patient outcomes.


Key Responsibilities


  • Develop and implement individualized patient physical therapy plans of care



  • Provide in-home visits, evaluating and reassessing patient needs



  • Communicate with physicians and care team members as needed



  • Deliver education and instruction to patients, families, and staff



  • Participate in interdisciplinary meetings to review and coordinate care



  • Ensure care plans are reviewed, updated, and documented accurately



  • Complete electronic documentation timely and accurately in EMR (WellSky/Kinnser preferred)



  • Adhere to all agency policies, state/federal regulations, and ACHC standards




Requirements


  • Current Physical Therapist (PT) license in the state of Florida (in good standing)



  • Specialized training and experience in Pelvic Floor Therapy



  • Knowledge of physical therapy principles, concepts, and methodologies



  • Excellent written, verbal, and interpersonal communication skills



  • Strong problem-solving and decision-making abilities



  • Ability to collaborate effectively with an interdisciplinary team




Required Credentials


  • Current CPR certification



  • Level 2 Background Screening



  • Recent negative TB test or chest X-ray
