What You'll Do

As a Physical Therapist, you'll work 1:1 with patients in their homes, helping them regain strength, mobility, and confidence. You'll collaborate closely with physicians, caregivers, and a compassionate interdisciplinary team.


Key Responsibilities


  • Create individualized treatment plans based on physician orders



  • Provide goal-oriented physical therapy in patients' homes



  • Educate patients and families to support independence and recovery



  • Document all visits accurately and promptly in our EMR system



  • Attend interdisciplinary meetings to ensure coordinated care



  • Follow all agency policies and regulatory requirements




What You'll Need


  • Active Florida PT license in good standing



  • Current CPR certification



  • Level 2 background screening



  • Recent negative TB test or chest X-ray



  • Solid knowledge of physical therapy best practices



  • Great communication & teamwork skills



  • 1+ year of PT experience (multidisciplinary setting preferred)



  • Experience with AlayaCare EMR is a plus
