This position is accountable for taking care of patients and completing all clinical responsibilities. Provides multidisciplinary team coordination services to the assigned areas. Provides medical education to residents, interns and other students. The incumbent participates in or contributes to an environment to advance medical research. The incumbent provides independent judgment in diagnosis and treatment of patients, formulation of treatment plans, and direction of the interdisciplinary team in achieving the goals of the patient care plan. Community Health and Wellness Center (CHWC)isin search of a passionate, dedicated, team-oriented Primary Care (IM or FM)Physician, looking to make a real impact in our community. Must have the ability and desire to thrive in a patient experience culture with attention to value of care. The Center provides continuity of care and transitional care for hospital discharges. CHWC is a "Medical Home" for adults needing healthcare when it's not an emergency. Interdisciplinary Team Care Model Includes: Physicians, NPs, PAs, LCSW, Clinical Pharmacist, Pharm Techs, Transition of Care Coordinator, Financial Counselor, Referral Coordinator, RNs, CMAs. Interest in Caring for the Underserved, many with complex chronic medical conditions. Onsite Health Education and Prevention Programs Onsite: Pharmacy, Point-of-Care Lab Testing, Food Pantry, and Clothing Pantry Excellent Call Structure with a Nurse Triage Service 4 1/2 days of Patient Care and 1/2 day of Administrative Time 40 Days of Paid Annual Leave; $5,000 Annually for CME; Professional Liability Insurance plus tail coverage.
