Overview RN Case Manager - Hospice (Lakeland, FL) Compassionate Care Hospice, an Amedisys Company This position is based in Lakeland, FL and requires daily travel to Bartow and/or Fort Meade. Final compensation may vary based on experience and skills. Technology Used:* Hospice RN Case Managers utilize and leverage: * HCHB = Proprietary Real-time database * Enclara Pharmacy solutions with 24/7 access * IPrescribe - Efficiencies with medication management * Forcura - Document management system * Medline - Supply ordering system * TalentEDGE - Amedisys' premier learning center * EBSCO - Nursing Resource Center for CEUs * Tablet with voice dictation and mileage tracking * Outlook added to Teams, Chat, Yammer Orientation, Onboarding & Training You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Responsibilities Performs patient assessments and collaborates with the care team to develop and implement a plan of care. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Performs on-call responsibilities and on-call services to patients/families as assigned. Other duties as assigned. Qualifications Current RN license, specific to the state(s) you are assigned to work. 1+ year(s) of clinical RN experience in acute healthcare setting(s) - REQUIRED. Current CPR certification.
