Overview

Assists in the coordination of activities associated with the VNS Health Community Center. Functions as a liaison between external customers, patients, families and VNS Health. Builds relationships, obtains referrals and reviews patient information referred to VNS Health to determine initial appropriateness of admission. Recruits and retains members and contribute to business growth. Works under general direction.

* Educates community members about all VNS Health lines of business.

* Provides a welcoming environment for visitors, offering comprehensive information to promote the Center's services and activities.

* Conducts outbound calls to members to promote Center's activities. Fosters relationships with members and identifies potential business opportunities.

* Promotes VNS Health services, including but not limited to CHHA, Hospice, Personal Care, and Health Plan. Follows through on various processes, including completing an appropriate referral form and submitting it to the appropriate unit and ensuring seamless transitions.

* Liaises with physician offices and referral sources to procure essential information, optimizing the intake process.

* Maintains detailed reports to track various referrals and admissions processed, ensuring accuracy and efficiency, and maintaining a high level of customer satisfaction.

* Liaison between consumers (e.g., patients, members, clients, and families) and customers (e.g., B2B, referral source, payers, and provider networks). Facilitates effective communication and referrals as needed.

* Resolves consumer issues by collaborating with relevant departments, ensuring prompt and practical solutions.

* Recruits new members and enters new membership information.

* Identifies business opportunities and assists potential members in setting up assessment appointments.

* Manages the daily administrative activities of the program, including maintaining the filing system, performing routine typing, photocopying, faxing, and handling mail management, etc.

* Conducts telephone outreach and surveys as needed, ensuring high levels of engagement and feedback.

* Maintains daily activity calendar, ensuring all events and activities are well-coordinated.

* Develops digital slides to promote centers and organizational events.

* Manages event venues and coordinates the logistical setup and breakdown of furniture for daily activities, ensuring efficient space utilization.

* Coordinates the work assignments for volunteers.

* Manages the inventory of Center's supplies, literature and equipment maintainance, coordinating service needs to ensure operational efficiency.

* Participates in and support community events. Ensures VNS Health's presence and visibility and actively recruit members.

* Liaison between the Sales/Member Service/Retention team and new enrollees to address any issues that arise after enrollment.

* Captures relevant data related to member dissatisfaction and disenrollment and provide internal feedback on improving member retention.

* Assists Health Plan members with Medicaid Recertification, DME & Medical Supplies request submission, surplus payment, OTC card activation, reviewing benefits, PCP assignment, and contact information verification, billing issues, and communicate this information to the relevant department promptly to ensure high level of member satisfaction with services.

* Participates in special projects and performs other duties as assigned.


Qualifications

Education:
High School Diploma or GED required Bachelor's Degree preferred

Work Experience:
Minimum two years of customer service experience required.

Proficient in speaking, reading, and writing Chinese.

Fluency in other dialect is preferred.

Experience in a healthcare organization or in social services preferred. Demonstrated knowledge of Microsoft Windows preferred.


Pay Range

USD $23.17 - USD $28.96 /Hr.
