A Brief Overview
The Clinical In Vivo Gene Therapy (CIGT) Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for the direct patient care of gene therapy recipients as well as systems development for these patients. They will be involved in the follow-up of a population of patients that matches his/her delineation of privileges and skill set. The components of the role include clinical practice, consultation, quality improvement, professional development, education, and research.

The CIGT APP will partner with the CIGT medical director to support the care of clinical and research patients receiving in vivo gene therapies. This position will have an integral role on the gene team consult service, working within a multi-disciplinary team and reporting to the CIGT medical director to coordinate and deliver high-quality care to those who experience gene therapy complications. Additionally, the CIGT APP will work with CIGT research leadership to help provide clinical support of industry and investigator initiated clinical trials, particularly around high-risk toxicity windows coordinating laboratory follow up.

The CIGT APP will be responsible for developing and maintaining relationships with internal and, as necessary, external providers to ensure clinical and operational effectiveness and may provide support of institutional initiatives with a focus on safety, quality, operational excellence, financial stewardship, patient-family experience, cultural awareness, and promotion of interdisciplinary collaboration as it relates to gene therapy recipients. This role will liaise closely with the CIGT medical director to support initiatives around best practices, QI, operational efficiency, long-term follow up of gene therapy recipients, and physician and staff training.

The ideal candidate will have:

  • 2-3 years of Nurse Practitioner or Physician Assistant experience required

What you will do

  • Develops and co-develops appropriate policies, pathways, and workflows for gene therapy patients supporting safe, high-value, and high-quality care. Collaborates to co-design and implement change in close collaboration with CIGT medical director.

  • Inpatient rounding and care coordination with as a key member of the multidisciplinary gene therapy consult team (the "gene team").

    • In this capacity, rounds on all patients with planned admissions for in vivo gene therapy as well as on patients who are admitted with complications daily (weekdays) as clinically indicated.

    • Collaborates with attending physicians and other clinical leaders/providers to ensure highly effective and efficient care for gene therapy patients and that the plan of care is properly documented.

    • Utilizes advanced assessment skills to manage a cohort of patients. Performs and documents daily assessments. Collects and analyzes data and formulates a family-centered plan of care

    • Evaluates and monitors the effectiveness of the prescribed Gene team treatment plan. Using the most current evidence, modifies the plan of care based on the effectiveness of the treatment and the patient/family response to the plan.

  • Maintains a list of active gene therapy patients (both clinical and research) with their associated labs and clinical courses

    • Organizes and leads a weekly gene therapy rounds meeting, where all active and upcoming patients are reviewed and presented

    • Assists with patient follow up with the primary teams following the weekly meeting

  • Participates in the care of research and commercial gene therapy patients, serving as a partner for clinical divisions administering gene therapy

    • Serves as a liaison between CIGT and clinical divisions implementing new gene therapy products. May embed within clinics for 2-3 months at a time in divisions launching clinical gene therapy products. Supports APPs and nursing staff in other divisions during clinical launches. Serves as a resource for other teams as needed.

    • Ensures clinical and research patients are appropriately added to the CIGT Healthy Planet database. Regularly reviews Healthy Planet database and tracks appropriate metrics.

    • Serves as an educational resource to the patient/family, nursing staff, and provider team. Acts as a consultant to multidisciplinary health care team members

  • Oversees and/or performs all aspects of clinical research, and serves as a key member of the CIGT research team

  • Understand good clinical practice (GCP) and regulatory compliance

  • Documents investigational product (drug/device) accountability.

  • Complies with institutional, federal, state, and sponsor policies, standard operating procedures (SOPs) and guidelines.

  • Serves in a leadership/supervisory capacity on clinical research team. Participates in hiring, supervision, and training of new staff as designated by CIGT leadership Acts as mentor to others for their professional development.

  • Participates in clinical research committees/activities. Represents CHOP on multicenter clinical research committees.

  • Demonstrates independent decision making, patient care, and organizational skills beyond that of Clinical Research Nurse.

  • Performs any needed advanced procedures that are approved as a part of the credentialing process, as required by individual study protocols.

  • Participates as needed/required by individual study protocols in the outpatient and inpatient evaluation of children who were enrolled in clinical research protocols.

  • Oversees gene therapy education of other CIGT team members (clinical research associates).

  • Coordinates regular educational conferences to enhance professional development. Serves as an educational resource for research teams outside of CIGT.

  • Ability to collect clinical data using quality improvement tools.

Licenses and Certifications
