RN Care Coordinator (Cardiology Clinic - Electrophysiology/CV Genetics)

As a Care Coordinator - RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum. Our heart specialists and their teams have been pioneering heart care research and innovative techniques used to treat heart disease for more than 60 years.

What you will do as an RN Care Coordinator:

• Coordinates care across the health care continuum within designated sub-specialty population(s)

• Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services

• Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits

• Facilitates proactive disease management and preventative health care using evidence-based practice

• Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:

• Current Minnesota RN license

• 3+ years of Clinical Nursing experience

• BLS certification

• Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy

Location: Clinics and Surgery Center - 909 Fulton St. SE. Minneapolis MN

Hours: 40 hours a week, 1.0 FTE
