SUMMARY STATEMENT & PURPOSE The Social Worker supports patients in meeting their psychosocial needs and achieving emotional health. Theyfacilitatetreatment planning, conduct assessments, providepsycho-education, offer short-term therapy, lead group sessions, and coordinate discharge planning with community resources. Qualifications Essential Job Functions Provide comprehensive psychosocial assessment, intervention, and support services to patients and their families. Conduct individual sessions, facilitate therapeutic and educational groups, and collaborate with interdisciplinary teams to develop, implement, and update individualized treatment plans. Serve as a resource to families regarding emotional, social, and financial aspects of care while ensuring patient and family engagement in treatment planning. Maintain timely, accurate clinical documentation, coordinate care across departments, verify required releases of information, and ensure compliance with regulatory standards and organizational policies.
