Forensic Care Coordinator

Dane, Jefferson & Walworth Counties, WI
Full-Time | Benefit Eligible | Hybrid Remote/Community-Based


Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a dedicated Forensic Care Coordinator to support individuals involved in Wisconsin's forensic mental health system. This role serves clients participating in the Opening Avenues to Reentry Success (OARS) program, Conditional Release (CR), and Competency Restoration programs, helping individuals successfully navigate treatment, community integration, and recovery. What You'll Do

  • Provide person-centered case management and care coordination for forensic clients.
  • Meet with clients in homes, community settings, correctional facilities, treatment programs, and other locations as needed.
  • Assess client needs and develop individualized service plans in collaboration with clients, families, providers, and support systems.
  • Coordinate and monitor services that support recovery, treatment adherence, housing stability, employment, and community integration.
  • Partner with treatment providers, courts, correctional institutions, county agencies, and natural supports to achieve positive outcomes for clients.
  • Maintain timely and accurate electronic documentation and records.
  • Facilitate team meetings, advocate for client needs, and participate in ongoing training and professional development.
  • Provide court testimony and written reports when required by program responsibilities.


Schedule & Work Environment

  • Primary schedule is Monday through Friday.
  • Flexibility is required to accommodate client needs, including occasional evenings and possible weekend responsibilities.
  • Hybrid work environment with a combination of:

    • Community-based client visits
    • Collaboration with community partners and treatment teams
    • Remote work when not meeting with clients


  • Daily travel throughout Dane, Jefferson, and Walworth Counties is required.


Qualifications
Required

  • Bachelor's degree in a human services-related field. Acceptable areas of study include:

    • Criminal Justice
    • Psychology
    • Social Work
    • Counseling
    • Rehabilitation Counseling
    • Community Mental Health
    • Substance Use Studies
    • Other related human services disciplines


  • Valid driver's license, reliable transportation, and ability to meet LSS driving requirements.


Preferred

  • Experience working with individuals impacted by mental illness, substance use disorders, criminal justice involvement, or community reentry.
  • Knowledge of trauma-informed care, person-centered planning, and community resources.
  • Strong communication, documentation, collaboration, and relationship-building skills.
