Description

The Senior Medical Director of Primary Care holds the accountability in a partnership with the Senior Director of Primary Care Operations for the delivery of all primary care services provided at Central City Concern. This role leads and mentors the organization's primary care clinical staff and sets strategic direction in partnership relating to all aspects of primary care services with the Senior Director of Primary Care Operations, the Chief Medical Officer, the Chief of Health Services Operations.

The Senior Medical Director of Primary Care also establishes clinical culture and promotes the professional, clinical, and ethical values and standards to which all Primary Care Clinical staff are expected to adhere. The Senior Medical Director of Primary Care promotes the quality and safety of care, establishes and maintains the organization's standards of clinical care, and has the responsibility for the clinical culture of service excellence, patient safety and continuous improvement.

Location: Blackburn Center (12121 E. Burnside St. Portland, OR 97216)

Compensation: $260,000 - $369,000, annually

Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity.

MINIMUM QUALIFICATIONS



  • License to practice medicine as a medical doctor or osteopathic physician, in Oregon.
  • Must have full prescriptive powers in Oregon with a DEA number
  • Current BLS certification or must obtain before date of start if not current.
  • Must meet CCC privileging requirements as required by FTCA.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988.An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Able to climb stairs several times a day.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations, and treat each individual with respect and dignity.


Experience



  • Minimum Four years' experience providing care or consultation in integrated primary care/behavioral health setting.
  • Minimum Four years' experience in established leadership capacity within a healthcare setting
  • Preferred: Minimum Seven years' experience in providing medical care as a medical provider for individuals in a Primary Care, Public Health.
  • Preferred: Clinical experience providing services and/or leadership within an FQHC and/or an HCH homeless clinic setting.
  • Preferred: Experience providing Medication Assisted Treatment (MAT) in a primary care setting with Buprenorphine/Naloxone or Buprenorphine.
  • Must have experience with prescribing buprenorphine or be willing to receive training in buprenorphine management with onboarding.


ESSENTIAL DUTIES AND RESPONSIBILITIES

Clinical Activity and Leadership



  • Provides direct patient care 0.4FTE at one or multiples sites.
  • Holds accountability for the cultivation and management of a workforce culture that drives for clinical performance of the primary care division to clinical service delivery targets and other quality and performance goals or targets identified by Executive Health Services Leaders
  • Serves as clinical leader for primary care Licensed Medical Providers (LMPs) and other primary care clinical staff at all of our primary care clinics, Mobile Medical, and Recuperative Care Center including availability for brief clinical consultation, provision of clinical teaching, clinician mentoring, and modeling of exemplary clinical practice.
  • Serves as the clinical leader of CCC primary care services and is engaged in the discernment, development and strategic planning required to successfully meet payer-based contractual priorities and performance goals.


Management and Supervision of CCC's Licensed Medical Providers



  • Manages and supervises primary care providers in all primary care professional and clinical domains, excluding providers on the behavioral health team.
  • Mentor and coach the LMPs he/she directly supervises in a manner that creates a culture of accountability, fosters continual improvement and staff wellness.
  • Provides medical direction and clinical supervision to CCC's primary care providers in accordance with state regulations and standards of practice. Ensures all providers receive appropriate staff development, in-service training and performance review.
  • Leads the development and teaching of standard work related to medical practice for LMPs based on best practice, continuous learning and improvement, and compliance with rules and standards of practice.
  • Assures, through an interdisciplinary process, the appropriate credentialing and privileging of all LMPs.
  • In dyadic partnership with the Senior Director of Primary Care and Pharmacy Operations and Clinics, leads the implementation of consistent care team models of care across the primary care continuum, including LMP roles for standardized best operational practices.


Assurance of Superior Quality of Medical Care



  • Responsible for the development and the representation and promotion of all medical practice guidelines, policies and protocols for Primary Care at CCC , Recuperative Care and Mobile Health through the Clinical Standards Committee.
  • Identifies, selects and oversees implementation of clinical evidence-based practices and "best practices" across CCC Primary Care, RCP and Mobile Health programs.
  • Supports the development of new clinical programming pursuant to clinical excellence in the provision of primary care services within CCC.
  • Responsible for clinical safety monitoring, ongoing clinical risk assessment, and promotion of just culture within CCC's Primary Care, RCP and Mobile Health programs.
  • In collaboration with other primary care leadership, responsible for monitoring, and improving/maintaining financial performance metrics within CCC's Primary Care programs.
  • Monitor and promote improvement or maintenance of local/federal clinical performance metrics and certifications (ie HRSA, NCQA, PCPCH, CCO) within CCC's Primary Care, RCP and Mobile Health programs.
  • Provides clinical leadership as a member of the CCC Controlled Substance Review Committee.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.


DESCRIPTION OF OTHER DUTIES



  • Participates in budget development and financial performance monitoring for CCC primary care programs.
  • Collaborates with CCC's Health Services Leadership teams to promote exemplary clinical care across Primary Care, Mental Health and Substance Use Disorder treatment services at CCC.
  • Collaborates with CCC's Housing and Employment leadership teams to foster culture of partnership and integration across CCC.
  • Attend all bi-weekly and bi-monthly meetings as needed.
  • Attends monthly Care Oregon Metro Clinical Workgroup meetings
  • Provides linkage to professional and academic communities pertaining to primary care and community health practice
  • Provides linkage to accrediting and monitoring bodies and agencies pertaining to primary care medical practice.
  • Other duties as assigned.

  • Knowledge, training, skills and experience in providing and supervising services in a highly diverse work environment to a highly diverse and mostly homeless population. Excellent communication skills including diplomacy, tact, effective "bedside manner" with patients.
  • Comfort with difficult or challenging conversations.
  • Ability to manage time and meet deadlines.
  • Demonstrated ability to effectively supervise health providers.
  • Demonstrated ability to design, implement, develop and manage an integrated clinical care model.
  • Ability to keep accurate and appropriate records/documentation.
  • Ability to display non-judgmental attitude.
  • Sufficient manual dexterity and physical ability to perform assigned tasks.
  • Excellent problem-solving and conflict resolution skills.
  • Knowledge of de-escalation methods or willing to be trained in these methods.
