The Home Health Services LPN provides, skilled nursing care for a group of patients usually in the setting of the patient's home under the supervision of an RN. Employs the nursing process to perform skilled care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the LPN provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Licensed Practical Nurse ensures adequate service provision, including scheduling of visits, based on patients' identified needs and priorities for care under the direction of an RN. Complies with all Home Care policies and procedures.

Work Location: Anne Arundel County

Work Hours: must be available from 8:30a-5p. However, patient visit times are flexible.

Who Should Apply:

• High School Diploma/GED or equivalent (Required)
• LPN - Licensed Practical Nurse license (Required)
• CPR - Basic CPR (Required)
• Prior work experience not required
• Valid Maryland Drivers' License and current auto insurance. Must have working vehicle, agreement to comply with Maryland Law for mandatory seatbelt use.
• 2-3 years of clinical experience strongly (Preferred)
• Previous home care experience (Preferred)
