About the Role
The Optum Care at Home (OAH) program provides longitudinal, integrated care to Optum's highest-risk patients in their homes. As a Nurse Practitioner, you'll work as part of an interdisciplinary team delivering comprehensive, hands-on care to medically complex patients, helping them avoid unnecessary hospitalizations and improve quality of life. This role is ideal for providers who enjoy autonomy, patient relationships, chronic disease management, and caring for patients in their home environment.
Key Responsibilities
- Conduct comprehensive in-home assessments
- Medical history review
- Medication reconciliation
- Physical examinations
- Vital signs and screenings
- Manage acute illnesses and chronic condition exacerbations
- Provide care during short-term rehabilitation episodes
- Document diagnoses and ICD-10 conditions accurately
- Collaborate closely with interdisciplinary care teams
- Lead advanced illness and advance care planning conversations
- Educate patients and caregivers on treatment plans and disease management
- Coordinate care and support services to improve outcomes
Required Qualifications
- Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)
- Active, unrestricted state NP license
- Active driver's license and reliable transportation
- DEA license/prescriptive authority (or ability to obtain)
- Ability to obtain collaborative practice agreement if required by state law
- Ability to travel extensively and perform home visits in a variety of settings
Preferred Qualifications
- 1+ years of NP clinical experience
- Experience in:
- Geriatrics
- Home health
- Long-term care
- Senior living settings
- Experience caring for medically and socially complex patients
- Knowledge of chronic disease management
- Experience with advanced illness and end-of-life discussions
- EMR and technology proficiency
