Part-time Advanced Practice Clinician (NP/PA), House Calls - Clallam County, WA

Role Type: Field-based, non-prescribing position providing annual in-home health assessments for Medicare Advantage and other health plan members.

• Conduct comprehensive in-home health assessments, including:
  • Medical history review
  • Medication reconciliation
  • Physical exams and vital signs
  • Preventive screenings and point-of-care testing
• Identify and document diagnoses to support care management
• Communicate findings to Primary Care Providers (PCPs)
• Address gaps in care and connect members with resources
• Educate patients on health conditions, medications, and treatment compliance
• Recognize urgent or emergent health issues and take appropriate action
• Assess social determinants of health and provide referrals when needed

• In-home, patient-focused care
• Preventive and wellness-focused rather than treatment-focused
• No prescribing responsibilities

• Collaborative work with PCPs and interdisciplinary care teams

• NPs: ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)
• PAs: Current NCCPA certification and state licensure
• Active, unrestricted state license
• Valid driver's license and reliable transportation
• Ability to perform assessments in various home environments
• Ability to meet state requirements for independent practice approval where applicable

• 1+ year of clinical experience (family practice, geriatrics, or home health preferred)
• EMR and technology proficiency
• Experience with geriatric or Medicare populations
• Ability to transport equipment up to 30 pounds and navigate stairs
• Bilingual language skills

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
