Sentara Medical Group has a full-time APP to join Sentara's Advanced Heart Failure Clinic in Norfolk, VA. Join our dedicated Physicians and APPs as they provide a team-based collaborative approach to patient care, and are offered seamless access to the many resources available throughout the greater Sentara network for their patients.

Position Highlights Include:
*Hybrid mix of Inpatient and Outpatient services for AHF Clinic
*Patient Panel of 15-17 patients a day
*Comprehensive Benefits and Retirement package

*$3,000 Annual CME allowance
*$5,000 Annual QPI Bonus

*Fully integrated EHR (Epic)
